Required Forms for Transportation Services

To ensure the safety and protection of all passengers, the following forms must be signed and returned prior to your child’s first ride:

1. Parental Transportation Consent Form – Grants permission for your child to be transported by our company and outlines pick-up/drop-off procedures.

2. General Liability Waiver – Releases the company from liability for incidents beyond our control and confirms you understand the safety policies in place.

3. Photo & Video Release Form – Allows the company to capture and use images or videos of passengers (for safety verification and/or promotional purposes).

Important:
These forms are mandatory and must be completed before services can begin. Bookings without signed forms cannot be confirmed.

Please complete and submit the forms online or in person at least 48 hours before the scheduled service.
